This notice covers the Fuzzy Media partner application form. Applications are reviewed by Brigham Mercadante at SLF Media Group.

What the form collects

Your name, email, location, business name if supplied, experience, equipment, goals, training needs, follow-through approach and any portfolio or introduction links you choose to share. Please do not include passwords, payment details or confidential client footage.

How it is used

Fuzzy uses these details to review partner applications, select an orientation path and contact you about the program. Submitting does not activate a partner account, enroll you in a paid plan or authorize you to onboard clients.

Storage and access

Submissions are stored in the site’s hosted application database. The application review screen is restricted to the designated owner. Basic submission-rate records and review changes are recorded to protect the form and track application decisions. Service providers supporting hosting and authentication process information as part of operating the site.

Links and optional materials

Linked portfolios and videos stay on their original services and are subject to those services’ access controls. Only share material you are authorized to share. No file upload is required.
